Constructing Your Entry Form

You can greatly simplify the chore of drafting an entry form by following these simple steps and suggestions.

First, put all the "PSA Requirements" in a dedicated section. Doing so will make cutting and pasting between Microsoft Word files very easy and, equally important, it will minimize the work of modifying the entry form in future years as requirements are modified and wording is changed.

Second, follow the suggested sequence of steps. Going through the process one step at a time makes it much easier and makes is less likely that errors and omissions will creep into the form.

Third, save the Word file and send it as an attachment along with your application and credit card application.

"Exhibition Standards"

"Entry Form Check List".

Cutting and pasting from the Microsoft Word file into your entry form eliminates typing, proof reading and omissions.

Step 4:

Entry Form must include
"PSA Recognition Statement"

1. You may use horizontal
or vertical format.

2. Size must be at least as large as that of other international organizations.

PSA Recognition Statement
Recognition Statement is required in entry form and catalog.


1. Right click on the image you wish to copy and paste into your document or "save image as".


2. Or left click on the image you wish to download.


3. Exhibition must use one of the PSA Recognition Statements in Entry Form and Catalog. Exhibition may choose either.

Size to presentation as needed.
